امیدوارم که حال شما خوب باشد. من و ریتا به نظر می‌رسد که چالش‌های اخیر سلامتی‌مان را به خوبی پشت سر گذاشته‌ایم و تنها با ویزیت‌های معمول پیگیری روبرو هستیم. ما بسیار خوش‌شانس هستیم که از پیامدهای بدتر در امان مانده‌ایم و خوشحالیم که زندگی‌مان به نوعی به حالت عادی بازمی‌گردد.

کاملاً با نظرات شما در مورد اهمیت تحقیقات علمی برای آینده درمان اعتیاد و مراقبت‌های پزشکی به طور کلی موافقم. عمیقاً متأسفم که آینده چنین تحقیقاتی در آمریکا در حال حاضر به دلیل نیروهای سیاسی که می‌توانند نقش برجسته کشورم را در پیشرفت‌های تحقیقاتی گذشته تضعیف کنند، تهدید می‌شود. مؤسسات علمی در سراسر آمریکا احساس می‌کنند که وجودشان و یکپارچگی علمی‌شان در خطر است. برنامه‌های محلی درمان اعتیاد نیز از کاهش بودجه پیشنهادی و تأثیر آن بر افرادی که خدمت می‌کنند و آینده سازمان‌هایشان نگرانند. این همه، حکمت موضع کنگره 60 در خودکفایی مالی و امتناع شما از وابستگی به بودجه خارجی را تأیید می‌کند.
اکنون، در نکته‌ای مثبت‌تر، مایلم شما را در جریان آخرین همکاری تحقیقاتی‌مان قرار دهم. جلسه‌ام با دکتر ساراپاس و کندیس موتون در هفته گذشته بسیار خوب پیش رفت. ما سؤالاتی در مورد بهترین روش‌های آماری برای مقایسه دو نمونه تحقیقاتی‌مان بررسی کردیم و به تصمیماتی رسیدیم که فکر می‌کنم بسیار خوب خواهند بود. احسان همچنان در ارائه پاسخ‌هایی که اطمینان می‌دهند آمار کنگره 60 را به درستی تفسیر می‌کنیم، کمک‌کننده بوده است. همچنین در حال بررسی انتشار نتایج کارمان در دو مقاله هستیم: یکی که تاریخچه، فلسفه، روش‌های درمانی و ساختار کنگره 60 را مرور می‌کند، و دیگری که دو نمونه جمعیتی ما را مقایسه می‌کند. نگرانی ما این است که انجام هر دو موضوع در یک مقاله ممکن است منجر به نگارش متنی بیش از حد طولانی برای انتشار در اکثر مجلات علمی شود. اگرچه پیش‌تر دو مرور کلی از کنگره 60 منتشر کرده‌ایم، این به‌روزرسانی دقیق‌ترین توصیفی خواهد بود که تاکنون برای مخاطبان بین‌المللی در دسترس قرار می‌گیرد.

سوم، درصد نوجوانان و جوانان در کنگره 60 بسیار کمتر از آمریکاست. آیا این به شروع دیرتر مصرف مواد مخدر و مشکلات مرتبط در ایران مربوط است؟ آیا درمان نوجوانان یا جوانان در مراکز درمانی جداگانه در ایران انجام می‌شود؟ آیا روش‌های درمانی خاصی در کنگره 60 برای اعضای جوان‌تر استفاده می‌شود؟
همچنین، اعداد و تاریخ‌های مختلفی در پیش‌نویس پروفایل کنگره 60 وجود دارد که با رنگ زرد مشخص کرده‌ام و می‌خواهیم قبل از ارسال نسخه برای بررسی در یکی از مجلات برجسته اعتیاد، آن‌ها را به‌روزرسانی کنیم. در آن زمان، شما می‌توانید کل مقاله را بررسی کرده و به‌روزرسانی‌های لازم را در بخش‌های مشخص‌شده انجام دهید. در ضمن، آیا می‌توانید تعداد اعضای کنگره 60 که در سال گذشته در آزمون راهنما شرکت کردند و تعداد افرادی که انتخاب شدند را تأیید کنید؟

Dear Hossein,
I hope you are doing well. Rita and I seem to have gotten through our recent health challenges in good shape and have only the usual follow-up visits to contend with. We are very fortunate to have escaped far worse outcomes and are pleased that our lives are getting back to some semblance of normal.
My congratulations to all the Observers who play such important roles within the leadership structure of Congress 60. It has been an honor for me to serve in one of these roles. My only regret is that I am not in Iran to visit the many branches of Congress 60 and to meet its members in person. It is a privilege to help disseminate the many distinctive features of Congress 60 to an international audience.
I could not agree more with your comments on the import of scientific research in to the future of addiction treatment and medical care in general. I am deeply saddened that the future of such research in the U.S. is currently being threatened by political forces that could erode the prominent role my country has played in past research advancements. Scientific institutions across the U.S. are experiencing what they perceive as a threat to their existence and their scientific integrity. Local addiction treatment programs also fear what proposed funding cuts will mean to those they serve and to their future as organizations. This all confirms the wisdom of Congress 60’s stance of financial self-sufficiency and your refusal to be dependent upon outside financial funding.
Now, on a more positive note, I want to update you on our latest research collaboration. My meeting with Dr. Sarapas and Candace Mouton went very well this past week. We explored questions of how to best statistically compare our two research samples and came to what I think will prove to be very good decisions. Ehsan continues to be helpful in getting us answers to ensure that we are interpreting the Congress 60 statistics correctly. We are also considering publishing the results of our work in two papers: one that provides a review of Congress 60’s history, philosophy, treatment methods, and governance structure, and a second one comparing our two population samples. Our fear is that doing both topics within the same article might require a manuscript too lengthy for publication in most scientific journals. Even though we published two earlier overviews of Congress 60, the update will be the most detailed description to date that will be available to international audiences.
There are some questions I wish to pose to you to help fill in the profile and to aid our discussion of the comparison of the Iran and U.S. samples.
First, we assume there may be differences in client needs within the branches of Congress 60 emerging from different member characteristics, drug choice trend differences across Iran, and differences in attitudes and recovery support resources in different areas of Iran. Do the methods of Congress 60 vary from branch to branch in response to such needs or are the methods the same across all branches?
Second, the Congress 60 sample is noteworthy for having low participation of women. Does this result from the separate women only treatment programs available in Iran? Is this influenced by any legal, cultural, or religious factors? Could you describe any special approaches to how addicted women are treated who seek recovery within Congress 60?
Third, the percentage of adolescents and young adults in Congress 60 is far lower that in the U.S. Is this related to later onset of drug use and related problems in Iran? Treatment of adolescents or young adults in separate treatment facilities in Iran. Are there any special treatment methods used in Congress 60 for younger members?
Also, there are various numbers and dates in the draft profile of Congress 60 that I am highlighting in yellow that we will want to update just before we submit the manuscript for peer review in one of the leading addiction journals. At that time, you can fact check the entire article and do updates where needed to those highlighted areas. In the meantime, can you confirm the number of Congress 60 members who most recently took the Guide examination this past year and the number who were selected?
