خوشحالم که میشنوم جلسه عمومی با مسافران و همسفران بهخوبی انجام شده است و اعلام نقش من بهعنوان دیده بان بینالمللی کنگره ۶۰ به گرمی مورد استقبال قرار گرفته است.
اینکه اجتماع کنگره ۶۰ من را برای این نقش پذیرفته است افتخار بزرگی است و هر آنچه بتوانم انجام خواهم داد که لیاقت اعتماد همه اعضا را داشته باشم. در انتظار ترجمه احسان از پیمان کمک راهنمایی هستم و شال را بهعنوان نمادی از تعهد مشترکمان به گسترش ریکاوری در سراسر جهان عزیز خواهم داشت. کنگره ۶۰ بر اساس ۳ راه شناختن بنا شده است.
راه اول دانش تجربی است که این اجتماع و متدهای آن چه معنایی برای زندگی مسافران و همسفران داشته است. آنهایی که تحت تأثیر این متدها و اجتماع تبدیل شدهاند، سندی زنده هستند که هیچکس نمیتواند کتمان کند.
ستون دوم شامل دانش کلینیکال متخصصان اعتیاد است که شاهد مؤثر بودن رویکرد کنگره ۶۰ به درمان و خدمات ریکاوری بودهاند. مدرکی بر این ادعا استفاده از متد دی اس تی و جنبههای دیگر کنگره ۶۰ توسط ارگانهای دیگر فعال در حوزه درمان اعتیاد است.
ستون سوم، دانش علمی است، که در همکاریهای مشترک با کنگره ۶۰ در حال ظهور است. مدرک آن مقالههای علمی است که تنهایی و یا با همکاری با دانشمندان دیگر منتشر میکنید. فراتر از آن مدرک، دانشگاهی است که در حال ساختوساز است و آموزههای کنگره ۶۰ را به نسلهای آینده از افرادی که میخواهند کمک کنند و نیز دانشگاهیان انتقال خواهد داد.
این برای یک ان جی او واقعاً کمیاب است که چنین قدرتمند باشد.
درباره تردید نیدا در استفاده از کلمه درمان برای اعتیاد صحبت کردید. تاریخی طولانی پشت این تردید خوابیده است.
در اواسط سال ۱۸۰۰ در آمریکا آنچه "درمانهای معجزهآسا" میخواندند رو به رشد بود. بعداً مطالعات نشان دادند که در این بستهها و بطریهای درمانی در حقیقت دوزهای زیادی از الکل، مورفین، کوکایین و یا هیدرات کلرال وجود داشته است.
و اینطور بود که ادعای درمان با کلاهبرداری از مصرفکننده و خانواده او ترکیب شد. حتی در دوره مدرن نیز برنامههای خصوصی در قبال دهها هزار دلار وعده درمان میدادند (مانند متادون، بوپرنورفین و نالتروکسان) که گاهی بهعنوان "درمان" برای اعتیاد به مخدر در ایالاتمتحده مطرح میشدند درحالیکه مطالعات نشان میداد که ریکاوری بسیار پیچیدهتر از چیزی است که این داروها بتوانند فراهم کنند.
تعداد برنامههای درمانی که دارو درمانی مدتدار را با حمایتهای روانشناسی و معنوی (جهانبینی) در یک اجتماع و یا فرهنگ ریکاوری مانند کنگره ۶۰ ترکیب کرده باشند در آمریکا بسیار کم است.

در مکاتبات اخیرم با متخصصان اعتیاد درباره کنگره ۶۰ دو سؤال مطرح شده است که اگر ممکن است میخواهم پاسخ شما را بدانم.
سؤال اول درباره گسترش سریع پزشکانی است که بوپرنورفین را بهعنوان درمانی برای اعتیاد به مخدر در آمریکا تجویز میکنند. از من پرسیدهاند که آیا این اتفاق در ایران هم در حال رخ دادن است و اینکه اوتی که در کنگره ۶۰ استفاده میشود چه مزایایی در قیاس با استفاده از متادون و بوپرنورفین گروههای درمانی دیگر دارد.
قبلاً بهطور خلاصه دراینباره صحبت کردهایم اما جواب جدید برای این سؤال میخواهم.
سؤال دوم ریشه در تجربهای در آمریکا دارد که نشان داده است که افرادی که روی متادون و یا بوپرنورفین قرار میگیرند در مرحلههای آخر کاهش دچار بازگشت به اعتیاد میشوند و یا وقتی این داروها قطع میشوند دوباره به اعتیاد روی میآورند.
سؤالی که پرسیدهاند این است که آیا کنگره ۶۰ حمایتهای خاصی برای افرادی که در مرحلههای آخر استفاده از اوتی هستند و ماههای بعد قطع آن در نظر میگیرد؟

Dear Hossein,
I am delighted to hear that the public meeting with travelers and co-travelers went well and that the announcement of my role as international Didehban of Congress 60 was warmly received. Being accepted in this role by the Congress 60 community is a great honor for me and I will do all I can to be worthy of everyone’s trust. I look forward to Ehsan’s translation of the Didehban pact and will cherish the shawl as a symbol of our shared commitment to spreading recovery throughout the world.
Congress 60 is built on three distinctive ways of knowing. The first is the experiential knowledge of what this community and its methods have meant in the lives of travelers and co-travelers. Those touched and transformed lives represent the “living proof” that no one can challenge or contest. A second pillar consists of the clinical knowledge of addiction professionals who witness the effectiveness of Congress 60’s approach to treatment and recovery support. This is evidenced by the adoption of the DST method and other aspects of Congress 60 by other addiction treatment organizations. The third pillar is the scientific knowledge emerging from the research being done in collaboration with Congress 60, as evidenced by the scientific articles that you are now publishing on your own and in collaboration with independent research scientists. It will be further evidenced by the university now under construction that will spread the teachings of Congress 60 to future generations of helpers and scholars. It is rare indeed for an NGO to have such a strong foundation of support.
You referenced NIDA’s hesitance to speak of “cure” of addiction. There is a long history in the U.S. behind such hesitancy. So-called “miracle cures” for addiction began being widely promoted beginning in the mid-1800s in the U.S. that were later discredited when studies revealed that most of these bottled and boxed cures were filled with excessive doses of alcohol, morphine, cocaine, cannabis, or chloral hydrate. And so claims of “cure” became associated with the criminal exploitation of addicted people and their families. Even in the modern era, disreputable for-profit private programs promised cures in exchange for tens of thousands of dollars. And particular medications (e.g., methadone, buprenorphine, naltrexone) have sometimes been touted in the U.S. as “cures” for opioid addiction in spite of research studies confirming that recovery is far more complex than what these medication along can provide. Few treatment programs in the U.S. have integrated time-limited medication support, psychosocial support within a vibrant recovery community/culture, and spiritual (worldview) support as happens in Congress 60 every day.
I will extend your best wishes to Dr. Seppala and Dr. DuPont. Our presentation preparation is nearly complete and will take place later in October.
In my recent communication with other addiction experts about Congress 60, two questions have come up that I would like your responses to.
The first is based on the rapid spread general practice physicians prescribing buprenorphine as a treatment for opioid addiction in the U.S. I have been asked if this is also occurring in Iran and what particular advantages OT offers within Congress 60 compared to the use of methadone or buprenorphine by other treatment providers. We have briefly discussed this before but I would like your current response to that question.
The second question stems from the experience in the U.S. where individuals maintained on methadone of buprenorphine experience heightened vulnerability for addiction relapse during the final stage of tapering from these medications or in sustaining recovery once this medication support has ended. The question I have been asked is if Congress 60 provides any special supports for individuals during the final stages of OT support and the months that follow.
I will be happy to pass along your responses to these questions.
